Acceptance of Terms
By scheduling an appointment or availing services at Vishalya Clinic, you agree to comply with and be bound by these Terms & Conditions. If you do not agree with these terms, please do not use our services.
Appointments and Scheduling
Appointments must be scheduled in advance by phone or in person. We request patients arrive at least 10 minutes before their scheduled time. Late arrivals may result in shortened consultation time or rescheduling. Cancellations must be made at least 24 hours in advance. Repeated no-shows may affect future appointment availability.
Consultation Fees
Consultation fees must be paid at the time of service unless prior arrangements have been made. We accept cash, UPI, and other specified payment methods. Fees are subject to change without prior notice. Receipts will be provided upon request for all payments made.
Patient Information
Patients or guardians must provide complete and accurate medical history, current medications, allergies, and relevant health information. Failure to disclose important medical information may affect treatment outcomes and we cannot be held responsible for complications arising from incomplete information.
Treatment and Medications
Treatment plans are developed based on clinical assessment and may be modified as needed. Patients must follow prescribed treatment protocols and dosage instructions. We are not responsible for outcomes resulting from non-compliance with medical advice. Prescriptions are valid as per regulatory guidelines and should be filled promptly.
Medical Records
Medical records are maintained confidentially as per applicable laws. Patients have the right to access their records upon written request. A reasonable fee may be charged for copies of medical records. Records will be retained as per legal requirements.
Consent for Treatment
By availing our services, patients or legal guardians provide consent for examination, diagnosis, and treatment. Specific procedures may require separate written consent. Parents or legal guardians must provide consent for treatment of minors.
Photography and Documentation
Clinical photographs may be taken for medical documentation purposes with patient consent. These images will be kept confidential and used only for medical purposes unless separate consent is obtained for educational or research use.
Referrals and Outside Services
We may refer patients to specialists, laboratories, or other healthcare facilities when necessary. We are not responsible for services provided by third-party healthcare providers. Patients are responsible for coordinating and paying for referred services directly.
Insurance and Billing
We may assist with insurance documentation, but patients are responsible for verifying coverage with their insurance providers. Patients remain ultimately responsible for all charges regardless of insurance coverage. Any disputes with insurance companies must be resolved directly by the patient.
Clinic Conduct
Patients and visitors are expected to maintain respectful behavior toward staff and other patients. We reserve the right to refuse service or terminate the doctor-patient relationship in cases of abusive behavior, non-compliance, or violation of clinic policies.
Communication
We may contact patients via phone, SMS, or email for appointment reminders, health information, or administrative purposes. Patients can opt out of non-essential communications by notifying us.
Liability Limitations
Our liability is limited to the direct medical services provided. We are not liable for any indirect, incidental, or consequential damages. This does not limit liability for gross negligence or intentional misconduct.
Modification of Terms
We reserve the right to modify these Terms & Conditions at any time. Updated terms will be effective upon posting at our clinic or notification to patients. Continued use of our services constitutes acceptance of modified terms.
Governing Law
These Terms & Conditions are governed by the laws of India. Any disputes shall be subject to the exclusive jurisdiction of courts in Barrackpore, West Bengal.
Severability
If any provision of these terms is found to be unenforceable, the remaining provisions shall continue in full force and effect.
